Life Financial Partners Online Application
IMPORTANT: You will need the following information to complete your application.
- Social Security Number/Employer Identification Number. It is required by Federal Law that LFP is authorized to make payment to legal and valid individual or entity only. SSN or EIN must correspond to your legal or registered name.
- Credit Card Information. You will be asked to provide your Credit Card Information to pay a nominal non-refundable administrative service fee as part of the application process. Please have your VISA, MasterCard, American Express or Discover card available. Make sure the name and address on the credit card match your name and address.
- Your application will not be submitted to LFP until your payment has been successfully processed.
- When done, please print a copy of the LFP Independent Agent Contract Agreement. It is recommended that you keep a printed copy for your record.
- All information must be completed by the applicant only.
